What does it mean to rap off-key?

When singing off key the singer is not in harmony with the song. Rappers when actually rapping and not trying to sing don't worry about the key of a song. The sound engineer may make some pitch adjustments if needed.
 
It means you're off-key. ...the notes your vocals are hitting are not in key with the melodies of the song.
