I have been recently asked by company permission to use my song for TV.
I agreed after doing some background check on them to make sure I could trust them.
After emailing each other back and forth I was sent a PDF from their music clearance and licensing department.
I understood all that was written and had no issues with signing it until I saw a spot that I had to fill showing "Copyright Share"
I make music just for fun and no nothing of the legal stuff behind everything so I was hoping someone could clear this up for me and also recommend me what percentage I should put.

Do you have a co-writer on your song? Are you signed to a publishing company, or do you have your own?
