How do you fix mistake on copyright form?

As far as I know you can't change your SL-35 form once you have sent it in. But if I were in your shoes and it was a minor change, maybe spelling or something, then I would call the copyright office and see if they can't flag it in their computer or something to make the change. I think the number is 707-3000 if I remember correctly, but of course it should be on the bottom of your copy.

If that doesn't work try the forms number, wait till the end of the options and then choose "operator" then from there you should be able to get to the right office. Here is that number (202) 707-9100
