Songwriting Split Sheet

I had a question about songwriting split sheets. Was wondering if you don't have a publisher your affiliated with, what do you put down for publishing on a split sheet? Do you just put your name?

Also, can anyone recommend some good split sheet pdfs to use?

any help appreciated. thanks
 
If you are actually signing an agreement that will impact a signed/released recording, and not just an "if-so"/future profits agreement, then you should already have your own publishing company set-up (if you are not signed to a publisher already). You can Google the process (pretty simple) for starting-up an artist-owned publishing company or check out info from ASCAP or BMI on the subject. This is important, because _IF_ you have some kind of a radio-hit, for example, ASCAP and BMI will not release the "publisher's share" of royalties to an individual; it must be a company that is in their data-base.
